Lockout, Tagout, Test — Before You Touch

Machines injure people during maintenance because energy was still there — electrical, hydraulic, gravity, springs. LOTO exists so that the person inside the machine is the only person who can bring it back to life.

The non-negotiable sequence

  • Identify every energy source — electrical, air, hydraulic, stored, gravity.
  • Isolate at the point of isolation, not the start button.
  • Apply YOUR personal lock and tag — one worker, one lock, one key.
  • Dissipate stored energy: vent air, release springs, block raised parts.
  • TEST before touch: try a start, verify dead with a tested meter.

Discipline that keeps it alive

  • Group work = group lock box; every person's lock on the box.
  • Never remove another person's lock — escalation goes through the responsible manager under written procedure.
  • Test your voltage tester on a known live source before and after proving dead.
  • A tag without a lock is a suggestion, not an isolation.
